The following constitutes Contest Rules for Radio Music Survey.com:
1. All Radio Music Survey.com contests are open to any/all
Radio Music Survey.com members, unless otherwise specified by Radio Music
Survey.com or its affiliates.
2. Only one winner per family or household will be allowed
each year.
3. Winners must sign a promotional release prior to receiving
their prize.
4. All unclaimed prizes will be forfeited at the end of 30
days.
5. Radio Music Survey.com is at liberty to give away any unclaimed
prize at the end of the thirty day grace period.
6. Odds of winning will depend on the number of qualified
entries.
7. Radio Music Survey.com may substitute another prize of
equal value, in the event of non-availability of a prize.
8. Employees of Radio Music Survey.com, Pinnacle Media Worldwide,
its advertising agencies, their affiliates, representatives, and their
families or households are ineligible to enter/win.
9. Method of shipping prizes will be at the discretion of
Radio Music Survey.com. Radio Music Survey.com is not responsible for
the loss of prizes by the shipper.
10. Prize manufacturer is responsible for quality according
to their warranties.
11. Additions or deletions to these rules may be made at
the discretion of Radio Music Survey.com, and may be changed at any time.
12. Winners must be 13 years of age or older.
13. Radio Music Survey.com, its agencies, affiliates, sponsors
or representatives absolve themselves of any liability, financial or otherwise,
resulting from any contests staged by Radio Music Survey.com.
14. All contests entries become the property of Radio Music
Survey.com. Furthermore, Radio Music Survey.com will not return any contest
materials crucial to the completion of the contest.
15. All contestants acknowledge, as a condition of entry,
that Radio Music Survey.com has a right to publicize, or publish the winner’s
name, city and state of residence, photographic image, character, likeness,
voice or all matters incidental herein.
16. All prizes are non-transferable and void where prohibited
by law. No cash substitution of prizes allowed.
17. Winners understand and agree that they are responsible
for any and all taxes incurred on prizes received.
18. In the event that a winner chooses not to accept a prize,
he or she forfeits all claim to that prize. Radio Music Survey.com then
has the right, at its discretion, to award that prize to a contest runner-up
or withhold the prize.
19. Basic contest rules and addenda are available for review
during regular business hours at the Radio Music Survey.com offices, 931
S Mission Road, Fallbrook, California, 92028.
20. Any drawings will be conducted by Radio Music Survey.com,
and the results of the drawing are final.
21. The decision of Radio Music Survey.com is final.
